she had a chronic rt lower limb ischemia 1 year ago and claudication more than 200 metres, 10 days ago acute thrombosis occurred e severe pain, diminshed motor power and hypoesthesia, cyanosis and delayed cap. refill. she had a ct angiogram revealing thrombotic occlusion of short segment(7-10cm) just distal to the common iliac bifurcation and patent femoral and distal arteries. left limb is good with good pedal pulses, she was planned for femoro femoral cross-over but we found that her cardiology consultation shows a very low ejection fraction of 37% with unknown cause, probably myopathy.
does she fits for the bypass now, is thrombolysis a better option and if she gets thrombolysis and saved the acute ischemia is the underlying lesion , stenosis, occlusion is amenable to angioplasty in these cardiac conditions.
